Why does GIST need KIT or PDGFRA mutation testing?
Because the mutation predicts which drug will work. The National Cancer Institute explains that most GISTs are driven by mutations in the KIT or PDGFRA gene, and imatinib targets that pathway — but certain PDGFRA mutations, such as D842V, resist imatinib and call for a different drug. Testing therefore decides both drug choice and dose before treatment even starts.
