The U.S. National Cancer Institute groups head and neck cancers by where they start — the oral cavity (mouth and tongue), the pharynx (nasopharynx, oropharynx and hypopharynx), the larynx (voice box), the paranasal sinuses and nasal cavity, and the salivary glands. Most begin in the squamous cells that line these moist surfaces, so they are usually called squamous cell carcinomas.
The NCI lists tobacco (including smokeless tobacco) and alcohol as the main risk factors, and infection with human papillomavirus — particularly HPV type 16 — as an important cause of cancers of the oropharynx. Epstein–Barr virus is linked to nasopharyngeal cancer. Diagnosis usually involves an endoscopic exam and biopsy, imaging such as CT, MRI or PET, and, for oropharyngeal tumours, an HPV/p16 test that helps guide treatment. Treatment options
The modalities the NCI lists for head and neck cancer, and an honest note on each:
- Surgery. Removal of the tumour, sometimes with neck lymph node dissection and, for larger resections, reconstruction. Some tumours can be reached with minimally invasive or transoral approaches; extent depends on the site.
- Radiation therapy. Central to many head and neck plans — often intensity-modulated radiation (IMRT) to spare healthy tissue — given alone, after surgery, or together with chemotherapy. Larynx and oropharynx plans often use radiation to preserve function. More specialised options such as proton therapy are limited and hospital-specific in Turkey; confirm availability directly with a named center.
- Chemotherapy. Cisplatin is commonly given concurrently with radiation (chemoradiation) for advanced disease, and sometimes before it. Cost scales with the number of cycles.
- Targeted therapy and immunotherapy. Cetuximab, an EGFR inhibitor, may be added to radiation in some cases; immunotherapy such as pembrolizumab or nivolumab is used for recurrent or metastatic disease. Which applies is decided by your pathology, not a template.
